When a patient's case goes beyond CFC’s scope of care, they are referred into the County Health Syatem. Our patient advocates provide one-on-one emotional support and direction to navigate this complex system. Dignity, confidence and expert guidance are extended when there are few, if any, friends or family to assist them. CFC patient advocates work with our patients until they are no longer needed.

Caring additional attention for identified patients allows us to go above and beyond in supporting their emotional needs. We have found this to be a key to a patient’s long-term successful recovery, especially because the lives of many of our patients can be exhausted and fragile from other life hardships before they arrive here.
